About This Form

This form does four things:

  • Collects your child's information so we can keep them safe

  • Makes sure you understand the risks involved in camp activities

  • Asks for your consent to emergency medical treatment if needed

  • Asks for your consent to photography (optional)

We've written it in plain language because we want you to actually understand what you're agreeing to — not just sign something without reading it.

Section 3 — Understanding the Risks

Rize Camp is designed to be fun, active, and safe. Our programs include entrepreneurship activities, team games, cooking, arts and crafts, and outdoor exploration at Fish Creek Provincial Park. All activities are supervised by trained staff.

That said, active camps involve real physical activity, and injuries can happen. By enrolling your child, you acknowledge that the following risks are a normal part of camp participation and cannot be completely eliminated:

  • Slips, trips, and falls during physical activities — possible sprains, bruises, or fractures

  • Minor cuts and scrapes from craft or outdoor activities

  • Insect bites, sun exposure, and heat during outdoor sessions at Fish Creek

  • Allergic reactions despite our best efforts to manage disclosed allergies

  • Accidental contact with other children during group games or activities

  • Weather-related conditions at outdoor locations

We take these risks seriously. Our staff are first-aid trained, we follow Alberta Camping Association standards, and we have emergency protocols in place. But we want you to understand that no amount of supervision eliminates all risk in an active, outdoor camp environment.

By signing this form, you confirm that you have read and understood these risks, and that you are enrolling your child knowing they exist.

Section 4 — Release of Liability

Please read this section carefully.

In consideration of Rize Camp accepting your child's enrollment, and for other good and valuable consideration:

As a parent or legal guardian, you agree, on behalf of yourself and your child, to release and not hold liable Rize Camp Inc., its directors, staff, volunteers, and contractors (collectively "Rize Camp") for any injury, loss, or damage arising from your child's participation in the camp program, including injury caused by the ordinary negligence of Rize Camp.

This release does not apply if injury is caused by gross negligence or willful misconduct by Rize Camp.

We understand this is a significant thing to agree to. We ask for it because accidents can happen in any active, physical program — and because we need to be able to run a great camp without the threat of legal action every time a child takes a tumble. Our staff work hard, they care about your kids, and this protection lets us focus on delivering the best possible experience.

Section 5 — Indemnity Agreement

By signing this form, you also agree to indemnify (reimburse) Rize Camp for any legal costs, settlement payments, or damages that Rize Camp incurs as a result of a claim brought on your child's behalf, if that claim arises from:

  • Your child's own actions or conduct during camp

  • Health information you failed to disclose or misrepresented in this form

  • A pre-existing condition that you did not inform us of

This agreement does not cover claims arising from Rize Camp's own gross negligence or misconduct.

Section 6 — Emergency Medical Authorization

In the event of a medical emergency, I authorize Rize Camp staff to:

  • Administer or arrange basic first aid

  • Call 911 and emergency services

  • Authorize emergency medical treatment if I cannot be reached

  • Transport my child to the nearest medical facility

Rize Camp will make every effort to contact you immediately in any emergency. Alberta Health Care (AHCIP) covers emergency medical services for eligible Alberta residents. Any costs not covered by provincial insurance are your responsibility.

Section 7 — Authorized Pickup

Only the following people are permitted to pick up my child. Rize Camp staff will not release my child to anyone not on this list, regardless of their claimed relationship.

If there are custody orders or legal restrictions affecting who can collect my child, I will provide Rize Camp with a copy before camp begins.

Section 8 — Photography Consent (Optional)

Rize Camp may photograph or film camp activities for use on our website, social media, and promotional materials.

This is entirely optional — your choice does not affect your child's enrollment or participation in any way.

Section 9 — Health Declaration

I confirm that:

  • My child is in good health and able to safely participate in camp activities as described above, subject to any conditions I have disclosed in Section 2

  • All medical, allergy, and medication information provided in Section 2 is complete and accurate

  • I will notify Rize Camp immediately if my child's health status changes before or during camp

  • I understand that withholding health information that leads to injury or illness may affect my ability to bring a claim against Rize Camp

Section 10 — Parent/Guardian Agreement & Signature

By signing below, I confirm all of the following:

  • I have read this entire form and I understand its contents

  • I am the parent or legal guardian of the child named above and have the authority to sign this form on their behalf

  • I understand and accept the risks described in Section 3

  • I agree to the Release of Liability in Section 4

  • I agree to the Indemnity Agreement in Section 5

  • I authorize emergency medical treatment as described in Section 6

  • All health and medical information in Section 2 is accurate and complete

  • I have indicated my photography consent preference in Section 8
